Consent to Conduct Business Electronically

Your consent to electronic records, disclosures, transactions and signatures is required in order to conduct business electronically with Plymouth Rock Group of Companies and any companies that are managed by a member of the Plymouth Rock Group of Companies. The Plymouth Rock Group of Companies include the following insurance companies: Plymouth Rock Assurance Corporation, Bunker Hill Insurance Company, Bunker Hill Insurance Casualty Company, Bunker Hill Preferred Insurance Company, Bunker Hill Property Insurance Company, Bunker Hill Security Insurance Company, Pilgrim Insurance Company, Mt. Washington Assurance Corporation, MAPFRE Insurance Company of New York, Palisades Safety and Insurance Association, Palisades Insurance Company, Palisades Property and Casualty Insurance Company, High Point Preferred Insurance Company, High Point Safety and Insurance Company, High Point Property and Casualty Insurance Company, Twin Lights Insurance Company, Teachers Auto Insurance Company of New Jersey, and Plymouth Rock Assurance Preferred Corporation for itself and as administrative servicer for 21st Century Advantage Insurance Company and 21st Century North America Insurance Company.

If you consent to electronic records, disclosures, transactions and signatures, you may withdraw that consent at any time. If you withdraw consent, a paper handling fee or premium change may apply. Withdrawing such consent does not withdraw any separate consent you have given for any form of electronic payments. Consent to conduct business electronically applies to all requested information the Company gives you or receives from you in connection with the Company’s service. If you consent to electronic records, disclosures and transactions, you agree that the permanent record of the information you have provided will be stored by the Company on its systems. After consenting to conducting electronic transactions, you may, upon request, obtain a paper copy of the transaction at no cost to you. If you have any trouble accessing any requested information on the Company’s website, or if you want to receive a paper copy of the record of the transaction, you should call the Company (Monday-Friday) or send an email to [email protected].

To access and retain records of your electronic transactions from the Company, you must be able to view the records on your monitor and print documents or send screen prints to your printer. Your consent does not mean that the Company must provide the requested information electronically. The Company may opt to deliver requested information on paper if it chooses to do so. The Company may also require that certain documentation from you be delivered to the Company on paper.

As part of creating your account on the self-service website, you will be asked to create a user ID and password, as well as provide answers to security questions. The user ID and password may be used to access your account and your personal information. You agree that the Company is entitled to rely on your user ID and password – any person using your user ID and password with your permission is authorized to have access to your account and personal information.

If you are requesting a quote from the Company on our website, you will be able to retrieve your quote information using your email address and other personal information you provided during the quote process. The Company will not be responsible for any loss or injury resulting from misuse of your personal information if you share it with others, provide others with permission to access your account or do not protect your login credentials. If you discover that there has been unauthorized access to your account you should call the Company immediately and change your user ID and password.

An electronic signature is a sound, symbol or process that you attach to a document or record indicating your intent to sign the document or record. Your electronic signature will represent your affirmation to the contents of the document or record and your affirmation that you have read the document or record.

Entire Agreement

This agreement constitutes the entire agreement between the Company and you and it supersedes all prior agreements, whether written or oral between the Company and you, relating to the subject matter hereof. I have read the foregoing information about conducting business electronically and consent to the use of electronic records, disclosures, transactions and signatures in connection with the use of the Company’s service in place of written documents and handwritten signatures.
